Care coordinators assist you and your family in the long-term care home admission process. When you apply for admission, you are encouraged to choose five homes. You will be offered the first one available.
Before you apply, be sure to tour the homes that interest you. It’s up to you to make tour arrangements by contacting the homes directly. Ask your care coordinator for information about long-term care homes in other counties in South West Ontario.
